Auditor feedback form

As a part of the U.S. Department of Justice (DOJ) initiative to improve PREA audit quality, the PREA Management Office (PMO) within the Bureau of Justice Assistance (BJA) and the PREA Resource Center (PRC) have implemented a formal process designed to gather information on complaints or concerns from the corrections field, DOJ-certified auditors, and other stakeholders about auditor conduct. This form was designed to encourage the submission of actionable information that the PMO and the PRC can use to guide ongoing audit quality improvement efforts.

Instructions:

Please use the form below to provide specific, factual information about the behavior or conduct of a specific auditor or auditors. None of the fields below are required in order to submit the form; however, you are strongly encouraged to include as much information as possible. You may submit this form anonymously, but please bear in mind that including more details will allow the PMO and PRC to more effectively follow up on the information you provide, with the goal of enhancing the PREA audit function.

Please be aware that neither the PMO nor the PRC has investigative authority. Information received through this form will become part of the permanent record on auditor activity and conduct. Any information on auditor conduct that alleges criminal conduct, malfeasance, negligence, fraud, or conflict of interest, and provides actionable information, will be referred to DOJ for consideration. DOJ-certified PREA auditors have an affirmative duty to report these activities and should use this form to fulfill this obligation.

Finally, please note that comments submitted on topics other than auditor conduct, such as comments on the standards, will not be processed.
