Audit Information Change Form

Auditors are required to inform the DOJ PREA Management Office and PREA Resource Center of any changes to their upcoming or in-progress audits by using the Audit Information Change Form. These updates include, but are not limited to, changes in onsite dates, changes in lead auditors, cancelled audits, updated contract information, etc.
