Michigan Department of Corrections PREA Sexual Abuse Retaliation Monitoring Form

This form was developed by the Michigan Department of Corrections to meet aspects of standard 115.67. The form was created to monitor staff and inmates who report sexual abuse or inmates who are an alleged victim of sexual abuse. The form is not used for monitoring staff or inmates who report an allegation of sexual harassment. Please take this into account when adapting for your own purposes.
